Antecedentes y objetivos:
El cáncer de pulmón no célula pequeña (CPNCP) en estadio IIIA-N2 es un grupo muy heterogéneo que engloba una amplia gama de diferencias en el tamaño y localización del tumor primario y la extensión de la afectación de los ganglios linfáticos mediastínicos. El tratamiento de los pacientes con CPNCP en estadio IllA-N2 potencialmente resecable es controvertido y la estrategia terapéutica óptima sigue sin estar clara. Aunque la quimiorradioterapia (CRT) definitiva es el tratamiento estándar en estos pacientes, el papel de la cirugía tras tratamiento de inducción, con quimioterapia (C T) exclusiva o quimiorradioterapia concomitante, se debate intensamente en la oncología torácica. Por tanto, el objetivo de la presente tesis doctoral, realizada en la modalidad de compendio de publicaciones, fue evaluar el papel de la cirugía en el tratamiento multimodal, analizar los factores pronósticos asociados con la supervivencia y valorar el esquema de tratamiento de inducción más óptimo en este subgrupo de pacientes.

Background and objetives:
Patients with stage IllA non-small cell lung cancer (NSCLC) comprise a highly heterogeneous group due to a wide range of differences in the size and localization of the primary tumor and the extent of mediastinal lymph node involvement. The management of patients with potentially resectable stage IllA- N2 NSCLC is controversial and the optimal therapeutic strategy remains unclear. Although definitive chemoradiotherapy (CRT) is a standard treatment option in these patients, the role of surgery following neoadjuvant treatment, exclusive chemotherapy (CT) or concomitant CRT, is intensely debated in thoracic oncology. In this context, the aim of this doctoral thesis, presented as a compendium of publications, was to evaluate the value of surgery in multimodality treatment, analyze the prognostic factors associated with survival and assess the optimal neoadjuvant treatment scheme in this subgroup of patients.
