Differences between Novice and Expert Raters Assessing Trunk Control Using the Trunk Control Measurement Scale Spanish Version (TCMS-S) in Children with Cerebral Palsy

dc.description.abstractThe Trunk Control Measurement Scale (TCMS) is a valid and reliable tool to assess static and dynamic trunk control in cerebral palsy. However, there is no evidence informing about differences between novice and expert raters. A cross-sectional study was conducted with participants between the ages of 6 and 18 years with a CP diagnosis. The TCMS Spanish version (TCMS-S) was administered in-person by an expert rater, and video recordings were taken for later scoring by the expert and three other raters with varying levels of clinical experience. The intraclass correlation coefficient (ICC) was used to evaluate reliability between raters for the total and subscales of the TCMS-S scores. Standard Error of Measurement (SEM) and Minimal Detectable Change (MDC) were also calculated. There was a high level of agreement between expert raters (ICC >= 0.93), while novice raters demonstrated good agreement (ICC > 0.72). Additionally, it was observed that novice raters had a slightly higher SEM and MDC than expert raters. The Selective Movement Control subscale exhibited slightly higher SEM and MDC values compared to the TCMS-S total and other subscales, irrespective of the rater's level of expertise. Overall, the study showed that the TCMS-S is a reliable tool for evaluating trunk control in the Spanish pediatric population with cerebral palsy, regardless of the rater's experience level.eng
dc.description.abstractLa Escala de Medición del Control del Tronco (TCMS) es una herramienta válida y confiable para evaluar el control estático y dinámico del tronco en la parálisis cerebral. Sin embargo, no hay evidencia que informe sobre las diferencias entre evaluadores no-expertos y expertos. Se realizó un estudio transversal con participantes entre 6 y 18 años con diagnóstico de parálisis cerebral. La versión en español de la TCMS (TCMS-S) fue administrada en persona por un evaluador experto, y se tomaron grabaciones de video para su posterior puntuación por el experto y otros tres evaluadores con diferentes niveles de experiencia clínica. El coeficiente de correlación intraclase (CCI) se utilizó para evaluar la fiabilidad entre los evaluadores para las puntuaciones totales y de subescalas de la TCMS-S. También se calcularon el Error Estándar de Medición (EEM) y el Cambio Mínimo Detectable (CMD). Hubo un alto nivel de acuerdo entre los evaluadores expertos (CCI ≥ 0.93), mientras que los evaluadores no-expertos demostraron un buen acuerdo (CCI > 0.72). Además, se observó que los evaluadores no-expertos tenían un EEM y CMD ligeramente más altos que los evaluadores expertos. La subescala de Control de Movimiento Selectivo exhibió valores de EEM y CMD ligeramente más altos en comparación con el total de la TCMS-S y otras subescalas, independientemente del nivel de experiencia del evaluador.
